US Retail, Inc. (a franchise of Pet Supplies Plus) now offers all employees an opportunity to enroll in the Convenient Care Plus medical membership program. This program is not insurance, but rather a membership that offers free generic prescription drugs, reduced cost at most walk-in medical clinics, free telemedicine, and much more.
Signup is simple and membership fees will be deducted from your regular payroll.
Plans:
- Employee and Employee + 1 — $6.92 per pay period
- Employee + 2 or more dependents — $13.85 per pay period
If you wish to enroll, simply download the enrollment form, fill it out, and return it to your manager — The current enrollment period ends on July 12, 2019. Coverage begins on August 1, 2019.

Please note: Coverage can be dropped at any time, but you must then wait 1 year before re-enrolling. For new hires, coverage can begin on the first day of the month after employment begins. Coverage can also be kept if no longer employed by USR. However, rates increase by 3x.
